Consent forms to receive flu shots at the School-Based Health Center have been sent home with elementary students and were mailed to homes of MS/HS students.
These forms are required to set up an appointment for a flu shot at the SBHC, which is open from 7:30 a.m.-3:30 p.m. Mondays, and from 8 a.m.-4 p.m. Tuesdays-Fridays.
