Important sports information!

Interscholastic sport participation is available to all Sherburne-Earlville Central School District students in grades 7-12. Participation by a student in athletics involves a certain degree of physical risk or injury. All athletes must be medically cleared before the beginning of practice each season.

The following summarizes our requirements:

Physical exam: Completed by the student’s primary care provider or the school health care provider. This is valid for a period of 12 continuous months from the date of the exam and must be accompanied by a signed and completed Health History form. A copy of the exam must be on file in the Health Office.

Accident reports: Should a student-athlete sustain an injury during participation of the sport, an accident report must be completed. Students must notify the coach when an injury occurs. If a health care provider removes the athlete from participation due to an illness/injury, the student must submit a written note from the health care provider. Likewise, once the student is able to return, a written release must be obtained from the health care provider and submitted to the school’s Health Office.

Concussions: All students participating in a contact/collision/impact sport must take the concussion baseline test. The student and a parent are required to sign the written consent form to participate in the concussion management program. This is offered to sixth-graders this spring.

Rescue medication: Students requiring “rescue” medication (e.g. Albuterol, EpiPen, etc.) must provide the medication in its original, labeled container and submit written permission from a parent/guardian and a health care provider.

Family ID: Sports registration is done through an online program called Family ID, which keeps track of your student’s information. Registration opens 30 days before the first day of a sport’s practice. Registered students will be contacted by their coaches before the start of the season.

Permission forms: Health Appraisal, Health History, Medication Authorization and ImPACT permission forms are available online at https://www.secsd.org/HSMSHealthOffice.aspx (Departments >> Health Office >> Middle and High School Office).
