Hepatitis B Vaccination Requirements
        

As of September 1, 2000, New York State Public Health Law, Section 2164, mandates that all children entering the seventh grade must be immunized against the Hepatitis B virus. Children who are not immunized properly or do not show proof of proper immunization will not be able to remain in school past 14 days of admission.

Your child may receive the immunizations at your family physician or a public health clinic. You may call the Chenango County Public Health office at 607-336-1661 or the Madison County Public Health office at 315-363-5490 for their clinic dates.

Students enrolled in the SBHC may receive the vaccinations in the clinic at no out-of-pocket cost to the parent. Call 674-8416 for more information or to schedule an appointment.

All written documentation must be provided to the Middle School Health Office.
